For many years, the people of Greater Sylhet have recognized the need for professional education and better healthcare services to support community development. During the British colonial period, local leaders were working hard to establish a medical college in Sylhet in order to improve the existing healthcare system.

In response to these efforts, the government of Assam agreed to set up a medical college in Sylhet, and construction work for the college and hostel began in 1934. However, due to the influence of certain powerful officials, the project was unexpectedly shifted to Guwahati. In time, the Assam government, under public pressure, announced through an official gazette that a medical school would be established in Sylhet instead of a full medical college. Eventually, the proposed building was repurposed and converted into a hospital in 1936. This hospital later played a key role in serving British and allied troops during the Burma Front in World War II.

In 1948, the hospital was further upgraded and transformed into a Medical School, with appropriate teaching staff and residential accommodation, to produce Licentiate Medical Faculty (LMF) doctors.

In 1962, under pressure from various quarters, the Pakistan government decided to introduce a five-year graduate program leading to the MBBS degree. However, the number of seats available was still far below what was needed for the growing population.
